Senior Community Mental Health Nurse in Complex Psychosis

Midlands Partnership NHS Foundation Trust is seeking a highly skilled and compassionate Senior Community Mental Health Nurse to join our Complex Psychosis Rehabilitation team. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals with complex psychosis.

Key Responsibilities:
  • Assess and develop individualized care plans to meet the unique needs of patients with complex psychosis.
  • Provide expert mental health nursing care and support to patients in their homes and community settings.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Contribute to the development and implementation of evidence-based practice guidelines and protocols.
  • Participate in quality improvement initiatives to enhance patient outcomes and service delivery.
Requirements:
  • Registered Mental Health Nurse with a valid NMC registration.
  • Proven experience in community mental health nursing, preferably in complex psychosis rehabilitation.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and multidisciplinary teams.
  • Strong assessment and planning skills, with the ability to develop and implement individualized care plans.
  • Ability to work autonomously and as part of a team, with a flexible approach to meet the changing needs of patients and services.
