About the Role
The Midlands Partnership NHS Foundation Trust is seeking a skilled Community Mental Health Nurse to join our team in Telford. As a key member of our Urgent Care Pathway, you will work 24 hours a day, 7 days a week, providing mental health assessment, treatment, and support to individuals in Telford & Wrekin.
Main Responsibilities
As a Community Mental Health Nurse, you will:
- Provide specialist assessment and care planning in conjunction with service users and carers
- Evaluate treatment plans and undertake home treatment
- Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues
- Co-ordinate the work of the Team

Requirements
To be successful in this role, you will need:
- To be a Registered Mental Health Nurse as defined by the Nursing and Midwifery Council and be active on their register
- Previous experience of working in the community
