Job Summary

The Midlands Partnership NHS Foundation Trust is seeking a skilled Mental Health Practitioner to join our team in Telford. As a key member of our community mental health team, you will work closely with individuals who require mental health assessment, treatment, and support. Your role will involve providing specialist assessment and care planning, evaluating treatment plans, and triaging referrals. You will also work in partnership with internal and external agencies to deliver high-quality care.

Main Duties

Assessing mental health crises and delivering safe and effective home treatment for an identified client group in accordance with that person's wellbeing and recovery action plan where one exists or creating such a plan where not.

Be a Registered Mental Health Nurse as defined by the Nursing and Midwifery Council and be active on their register.
