Job Summary

The Midlands Partnership NHS Foundation Trust is seeking a skilled Community Mental Health Nurse to join our team in Telford. As an integral part of our Urgent Care Pathway, you will work 24 hours a day, 7 days a week, providing mental health assessment, treatment, and support to individuals in Telford & Wrekin.

Main Duties

As a Community Mental Health Nurse, you will:

  • Provide specialist assessment and care planning in conjunction with service users and carers
  • Evaluate treatment plans and undertake home treatment
  • Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues
  • Co-ordinate the work of the Team
