In this role, you will coordinate care services, working collaboratively with our multidisciplinary team to develop and implement individualized care plans. Key responsibilities include:
- Developing and implementing care plans that meet resident needs and preferences.
- Coordinating resident transitions, including discharge planning, medication management, and communication with next of kin.
- Monitoring resident well-being, identifying changes in needs, and ensuring timely updates to care plans.
- Promoting a culture of safety, through effective risk assessment and implementation of safe systems of work.
Requirements:
To succeed in this role, you must have:
- An NVQ 3 or equivalent qualification in care for care homes.
- Two years of experience in a senior position within a care home environment.
- Excellent communication and leadership skills.
- The ability to lead and motivate staff, acting responsibly in the manager's absence.
