Care Team Coordinator Job Description

We are seeking a Care Team Coordinator to join our care team and play a key role in delivering high-quality personal care to residents in our care home environment.

The ideal candidate will have experience in care coordination, team management, and inspiring clinical excellence. They will also possess strong communication and interpersonal skills, as well as the ability to build relationships with residents, their families, and external teams.

As a Care Team Coordinator, you will be responsible for coordinating care activities, managing staff rotas, and maintaining accurate records. You will also provide training and supervision to new staff members and assist residents with personal care duties.
