Leader for Community Transformation:

We are looking for a dedicated professional to drive our community transformation changes in East Sussex. Our goal is to develop Neighbourhood Mental Health Teams that provide innovative care pathways for older adults with mental health needs and/or dementia.

About the Job:
  • Prior experience in supervising and motivating staff is required.
  • Collaborate with local service providers to deliver high-quality, joined-up care for older people with mental health needs and/or dementia.

The role involves working closely with Clinical Academic Groups that lead on best practice and research related to care pathways, as well as the Centre for Dementia Studies, a joint venture between Brighton and Sussex Medical School and Sussex Partnership NHS Foundation Trust.

Responsibilities:
  1. Provide professional leadership to multi-disciplinary teams in delivering assessment, treatment, and interventions, including intensive support and treatment.
  2. Develop and implement effective care pathways for older adults with mental health needs and/or dementia.

Estimated salary: £94,500 - £117,000 per annum. The role offers opportunities for professional growth and collaboration with leading experts in the field.
