Role Overview
We are seeking a highly motivated and enthusiastic Clinical Health Practitioner to join our 0-19 Integrated Early Help Team in East Sussex.
The successful candidate will be delivering the National Healthy Child Programme, focusing on enhancing the health and wellbeing of children (aged 0-5 years) and their families.
This role involves working directly with families in their homes or clinic settings, addressing their needs in a holistic and impactful way.
Key Responsibilities:
- Deliver the National Healthy Child Programme to promote healthy lifestyles and prevent ill health in children aged 0-5 years.
- Work closely with families to identify and address their needs, providing emotional support and signposting to relevant services.
- Conduct Emotional Wellbeing Visits to parents who are needing extra support with their mental health in the perinatal period.
- Prioritise professional development through regular supervision and mentorship from experienced practitioners.
Requirements:
- A registered adult, paediatric, mental health nurse, or midwife with experience in public health.
- An understanding of the Healthy Child Programme and its implementation in community settings.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with families and colleagues.
