We are looking for an experienced Intensive Support Practitioner Team Lead to join our team and contribute to the development of our Neighbourhood Mental Health Teams. These teams will provide assessment, treatment, and interventions for older adults with mental health needs and/or dementia, aiming to prevent hospital admissions and provide care at home.

About the Role
  • Provide professional support and supervision to staff, as well as motivation and leadership skills.
  • Develop the intensive support function of the team while supporting the community transformation agenda.

The successful candidate will have experience in providing intensive support and treatment to individuals with mental health needs and/or dementia. The post offers access to local clinical and professional supervision, regular training, and personal development opportunities.
