The Community Heart Failure Nurse Specialist role at Sussex Community NHS Foundation Trust presents an exciting opportunity for an experienced professional to contribute to delivering high-quality patient care and improving health outcomes in East Locality.

This challenging yet rewarding position requires a highly skilled nurse with extensive experience in managing complex heart failure patients, who can provide expert guidance to other team members and support the development of community services.

About the Position:

The successful candidate will work collaboratively with the primary and secondary care heart failure teams, as well as Cardiologists, to ensure comprehensive patient care and develop effective strategies to reduce hospital admissions. The role demands a strong understanding of Long-term Condition issues, including NICE Clinical Guidelines and local CCG Commissioning Framework guidelines.

Main Responsibilities:
  • Provide expert guidance and support to other team members to enhance their skills and confidence in managing heart failure patients.
  • Develop and implement evidence-based interventions to improve patient outcomes, focusing on prevention and early intervention strategies.
  • Stay up-to-date with the latest research and best practices in Long-term Condition management, ensuring the delivery of high-quality patient care.
