About the Role

We are seeking a highly skilled and experienced practitioner to join our team as an Integrated Offender Health Specialist. This is an exciting opportunity to work with our Health and Justice service, supporting individuals with complex needs and vulnerabilities in the criminal justice system.

Key Responsibilities

  • Assess and develop care plans for individuals with diverse needs, working across the IOH Model, including Custody, Court, Community, and Prisons.
  • Collaborate with health and social care professionals to provide specialist assessment and interventions, ensuring effective care and support for individuals with acute or complex needs.
  • Act as an experienced practitioner, assessing, formulating, implementing, and agreeing care and support plans with adults and carers.
  • Provide consultation, support, and clinical supervision to other health and social care professionals within the service.
