About the Role

We are seeking a skilled and experienced practitioner to join our Integrated Offender Health (IOH) Service in Staffordshire. As a key member of our team, you will work closely with colleagues to provide specialist assessment and interventions to individuals with complex needs coming into contact with the Criminal Justice System.

Key Responsibilities

  • Assess and implement care and support plans for adults and carers
  • Work collaboratively with health and social care professionals to provide holistic support
  • Develop and maintain relationships with criminal justice partners to ensure effective referrals and support
  • Contribute to the development and implementation of service strategies and policies

Person Specification

We are looking for a qualified professional with a relevant degree or diploma in a field such as nursing, occupational therapy, or social work. You will have experience of working in a similar role and be able to demonstrate your skills and knowledge in assessing and implementing care and support plans.
