About the Role

We are seeking a skilled and experienced practitioner to join our Integrated Offender Health (IOH) Service in Staffordshire. As a key member of our team, you will work closely with colleagues to provide specialist assessment and interventions to individuals with complex needs coming into contact with the Criminal Justice System.

Key Responsibilities

  • Assess and formulate care and support plans for adults and carers
  • Implement a wide range of care services for individuals with acute or increasingly complex needs
  • Provide consultation, support, and clinical supervision to other health and social care professionals
  • Work collaboratively with criminal justice partners to ensure health requirements and vulnerabilities are addressed

Person Specification

We are looking for a practitioner with a relevant degree or diploma level qualification in Nursing, Occupational Therapy, or Social Work. You will have experience of working in a similar role and be able to demonstrate your skills and knowledge in assessing and implementing care plans.
