About the Role

We are seeking a skilled and experienced practitioner to join our Integrated Offender Health (IOH) Service in Staffordshire. As a key member of our team, you will work closely with colleagues to provide specialist assessment and interventions to individuals with complex needs coming into contact with the Criminal Justice System.

Key Responsibilities

  • Conduct comprehensive assessments of individuals' needs, identifying opportunities for care and support.
  • Develop and implement care plans, working collaboratively with health and social care professionals.
  • Provide consultation, support, and clinical supervision to colleagues within the service.
  • Work across various settings, including custody, courts, community hubs, and prisons, to ensure seamless care coordination.

Person Specification

We are looking for a qualified professional with a relevant degree or diploma in a field such as nursing, occupational therapy, or social work. You will have experience working with complex needs populations and a strong understanding of the IOH model. Excellent communication and interpersonal skills are essential, as you will work closely with colleagues and stakeholders to deliver high-quality care.
