This exciting opportunity is based in NHS Highland's new Community Link Worker service, where you will work collaboratively with GPs, Practice Nurses, and other healthcare professionals to develop strong links between Primary Care services and community resources.
Your Role:
- Support clients with complex mental health needs, utilizing Personal Support Planning to achieve personalized goals.
- Establish and maintain effective relationships with statutory and non-statutory agencies to facilitate coordinated care.
- Contribute to a multidisciplinary team, sharing knowledge and expertise to enhance client outcomes.
