Key Responsibilities:
- Develop and maintain positive relationships with clients, their families, and other stakeholders.
- Conduct assessments and create personalized support plans to address client needs.
- Work collaboratively with other professionals to ensure a seamless service delivery.
Required Skills and Qualifications:
- Degree or equivalent in Social Work or a related field.
- PVG membership is essential for this role, with the cost covered by Sacro.
