Role Overview
">Changemh is seeking a Community Link Worker to join its team in the Highlands. The successful candidate will work closely with GPs, Practice Nurses, and other members of Primary Care GP Practices or Clusters to establish links to community resources.
About the Position
">This is a varied role that requires strong communication and interpersonal skills, as well as the ability to plan and prioritise a workload. As a Community Link Worker, you will be responsible for a caseload, using Personal Support Planning to collaborate with individuals and help them achieve their health goals.

- Liaise with statutory and non-statutory agencies
