Key Responsibilities

As a Social Care Link Worker with Changemh, you will play a crucial role in supporting individuals affected by mental health or mental welfare issues. Your primary responsibilities will include:

  • Developing and maintaining relationships with local communities, healthcare providers, and other stakeholders.
  • Supporting individuals to improve their mental and physical health outcomes through Personal Support Planning.
  • Working closely with GPs, Practice Nurses, and other healthcare professionals to ensure seamless delivery of our services.
