Senior Nurse Practitioner Service Lead

The Hospital Discharge Team has an exciting opportunity for a skilled and motivated registered nurse or occupational therapist to assist with managing and supporting complex discharges. This role involves using a patient-centric approach to completing nursing assessments and formulating discharge plans.
You will work with Specialist Nurses and Physiotherapists, enabling you to expand your knowledge in multidisciplinary complex care. You will need to demonstrate flexibility, good time management skills, and excellent communication and interpersonal skills.
This is an exciting and fast-paced team, where each day can present new challenges and learning opportunities.
