About the Role:

We are seeking a highly skilled and experienced Senior Nurse Lead to join our team at Devon Partnership NHS Trust. As a key member of our multidisciplinary team, you will be responsible for assessing eligibility criteria under the NHS National Framework for Continuing Healthcare and Funded Nursing Care 2009 (revised 2022), planning and commissioning packages of care, and supporting individuals, their families, and health professionals through the Continuing Healthcare process.

Main Responsibilities:

  • Assess eligibility criteria for Continuing Healthcare and Funded Nursing Care
  • Plan and commission packages of care
  • Support individuals, their families, and health professionals through the Continuing Healthcare process
  • Carry a caseload of people with complex physical and mental health needs
  • Review and monitor packages of care at least annually

Requirements:

  • Sound understanding of the Continuing Healthcare Framework, including Funded Nursing Care and Section 117 aftercare
  • Knowledge and understanding of Mental Health and Mental Capacity Acts
  • Ability to work alone and as part of a team
  • Ability to work across a large rural area and different services
  • Skilled at providing clinical, professional, and management supervision
