About the RoleYour key responsibilities will include:
- Conducting thorough assessments to identify individual's strengths and challenges
- Developing and maintaining comprehensive support plans that prioritize independence and community engagement
- Collaborating with other professionals, such as therapists and healthcare providers, to ensure seamless care coordination
- Providing emotional support and guidance to individuals, their families, and carers
