Job Description
A Support Coordinator role is available with Home Group, with a salary range of £24,664 to £26,123 per annum and additional benefits. This position involves working collaboratively with colleagues to create person-centred support plans, focusing on supporting customers with complex mental health needs.

Responsibilities
Starting each day with a brief meeting to discuss ongoing cases and tasks, you'll then focus on delivering tailored support to customers. This may include assisting with daily living activities, helping customers access local amenities, and arranging social events.

Requirements
To succeed in this role, you'll need a strong understanding of mental health support and previous experience working in a similar environment. You must be able to use technology effectively to document customer interactions and collaborate with colleagues.
