Job Title: Support Coordinator

About the Role: We are looking for a Support Coordinator to join our team at Home Group Limited. As a Support Coordinator, you will play a key role in supporting our customers with complex mental health needs to live more independently in the community.

Key Responsibilities:

  • Supporting customers with their identified support needs, including helping with daily living activities, accompanying them to appointments, and assisting with budgeting.
  • Assessing customer referrals and developing person-centred support plans.
  • Working collaboratively with colleagues to achieve the best possible outcomes for our customers.

Requirements:

  • Experience of working in a supported housing or mental health background.
  • Ability to work on your own initiative and remain calm under pressure.
  • Enhanced DBS check and ability to use technology for creating and updating support plans.
