Job Description:

In this role, you will be responsible for developing and implementing personalized care plans, conducting assessments, and collaborating with external agencies to ensure comprehensive support. Our ideal candidate will possess a deep understanding of trauma-informed care, child development, and attachment theory, with excellent communication and interpersonal skills.

Duties and Responsibilities:
  • Develop and implement tailored care plans for young people with complex needs
  • Conduct thorough assessments and create reports on the needs of our young people
  • Collaborate with external agencies to provide seamless support
  • Foster strong relationships with young people, families, and colleagues

Estimated Salary: £44,000 - £54,000 per annum
