About the Role

We are seeking a highly skilled and experienced Frailty Specialist Nurse to join our team at Calderdale and Huddersfield NHS Foundation Trust. As a key member of our Acute Frailty Service, you will play a vital role in delivering high-quality patient care and contributing to the development of our service.

Key Responsibilities
  • Act as a specialist clinician for the Acute Frailty Service, providing expert care and guidance to patients and colleagues.
  • Work autonomously within Trust policies and professional practice, maintaining accurate and comprehensive records.
  • Manage your own specialist caseload, prioritizing tasks effectively and demonstrating excellent clinical skills.
  • Develop and implement the core components of the specialist Frailty nurse role, including clinical practice, management, and education.
  • Act as a clinical role model, promoting best practice in all assessments and providing leadership and guidance to staff.
  • Support the Team Leader in the day-to-day running of the Acute Frailty service, contributing to the development of our service and promoting a culture of excellence.
  • Participate in multidisciplinary teams, providing specialist advice and information to colleagues, patients, and carers.
  • Contribute to the assessment, planning, implementation, and evaluation of individualized programs of care for frail older adults.
  • Recognize changes in patients' conditions, referring on as appropriate and promoting a culture of safety and quality.
  • Present patients' perspectives, promoting and protecting their interests and dignity.

Requirements
  • First-level registration with the NMC.
  • Post-basic qualification in a relevant specialty.
  • Post-registration education in a relevant specialty.
  • Demonstrable experience as a band 5 in an elderly nursing environment.
  • Experience working in an acute environment.
  • Experience of palliative care.
  • Evidence of continued professional development.
  • Training and/or further study in elderly nursing.
  • Knowledge of local and national initiatives/strategies relating to elderly medicine and/or frailty.
  • Knowledge of community-based services, health, and care delivery.
  • Knowledge of using a holistic approach to facilitating patient-centered care.
  • Excellent written and verbal communication skills.
  • Ability to foster multidisciplinary working.
