About the Role

We are seeking a highly skilled and experienced Occupational Therapist to join our Acute Frailty Service as a Specialist Frailty Occupational Therapist. This is an exciting opportunity to work in a rapidly developing service that provides comprehensive geriatric assessments to frail patients presenting to our acute areas.

Key Responsibilities
  • Assess frail patients across our trust, including the emergency department, medical assessment unit, and Frailty Same Day Emergency Care unit (FSDEC).
  • Utilize core occupational therapy skills to support the complex needs of frail patients.
  • Work within a multi-disciplinary team to provide holistic assessments and meet the needs of our patients with a strong focus on the home first approach.
  • Develop and implement individualized treatment plans for frail older adults.
  • Act as a clinical role model, ensuring best practice in all assessments and providing specialist advice and information to professional colleagues, patients, carers, and the public.
  • Participate in the assessment, planning, implementation, and review of care pathways and transfers of care.
  • Work collaboratively with health and social care community and acute colleagues to ensure effective care pathways and transfers of care.
Person Specification
  • Registered with the Health and Care Professions Council (HCPC).
  • Relevant degree or diploma in Occupational Therapy.
  • Recent and extensive clinical experience working with frail patients, including assessments, complex and autonomous decision-making, and discharge planning.
  • Awareness of relevant legislation, National Service Frameworks, and local and national initiatives.
  • Excellent clinical reasoning skills in situations of complex health and social needs.
  • Evidence of continuing professional development, self-directed learning, and reflective practice.
  • Experience in dealing with risk assessment, supporting implementation of strategies to minimize risk, and reporting incidents and near misses.
  • Knowledge of best practice in relevant clinical areas and understanding of clinical governance.
  • Critical appraisal skills and ability to recognize the emotional demands of the post and demonstrate the necessary resilience.
  • Ability to recognize own limitations and have a willingness to share and learn skills from others.
  • Excellent time management and previous NHS experience either as an undergraduate or postgraduate.
