Job Title: Support Worker - Mental Health

We are seeking a dedicated and compassionate Support Worker to join our team at Oakwood House, a supported living community service in Stoke-on-Trent. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals with mental health problems, learning disabilities, and autism.

Key Responsibilities:
  • Assist with the assessment of individual needs and develop personalized care plans
  • Deliver care and support to individuals, promoting independence and well-being
  • Work collaboratively with the senior team to ensure seamless care delivery
  • Maintain accurate records of care provided and communicate effectively with stakeholders
Requirements:
  • Level 2 Health and Social Care qualification (desirable)
  • Experience working with individuals with mental health problems, learning disabilities, and autism (desirable)
  • Excellent communication and interpersonal skills
  • Ability to work flexibly and as part of a team
