**Job Title:** Healthcare Professional - Community Setting

**About the Role**

This is an exciting opportunity to work as a skilled nursing professional in a community setting, providing high-quality care to patients in their homes and local clinics. Our team at NHS provides community health services to over two million people across eleven London boroughs and Hertfordshire.

As a Healthcare Professional, you will be working under the supervision of our experienced Community Nurse Team Lead and Deputy Team Lead. Your primary responsibility will be to provide skilled nursing care to patients with complex health needs, ensuring continuity of care following referral from acute care, rehabilitation units, GPs, and other agencies.

**Key Responsibilities: **

  • To deliver high-quality nursing care based on principles of best practice and clinical evidence, including chronic disease management, end-of-life care, continence promotion, and wound management.
  • To undertake comprehensive assessments of patients' physical, psychological, and social care needs, including carers' needs, and develop individualised care plans that promote self-care and prevent unnecessary hospital admissions.
  • To act as a patient advocate, facilitating patients' choices regarding nursing care and promoting independence and self-care.
  • To participate in the implementation and monitoring of clinical and operational guidelines and policies, ensuring adherence to the Trust's Organisational and Clinical Guidelines and Policies.

**Requirements:**

  • Registered Nurse (RN) qualification with NMC registration.
  • Evidence of post-registration education and training.
  • Recent clinical nursing experience in a multidisciplinary team environment, with a focus on community nursing.
  • Knowledge of out-of-hospital care, infection control, general wound care, and recent government legislation changes in community care delivery.
