Job Summary

We support our patients at every stage of their lives, providing health visiting for new-born babies through to community nursing, stroke rehabilitation, and palliative care for people towards the end of their lives.

Job Responsibilities:

Main Duties:

  • To provide skilled, effective, and evidence-based care to patients with a variety of presentations.
  • To identify, prioritise, and implement programmes of care that meet the needs of clients/patients using an appropriate nursing model.
  • To undertake activities that promote health, reduce risk, and disease prevention.
