Nurse Practitioner

We are looking for a skilled nurse with excellent communication skills to join our team as a Nurse Practitioner - Community Care. The successful candidate will be responsible for providing high-quality care to patients in their own homes, working closely with other healthcare professionals to achieve optimal outcomes.

We support our patients at every stage of their lives, providing health visiting for new-born babies through to community nursing, stroke rehabilitation, and palliative care for people towards the end of their lives.
Job Responsibilities:Main Duties:
- To provide skilled, effective, and evidence-based care to patients with a variety of presentations.
- To identify, prioritise, and implement programmes of care that meet the needs of clients/patients using an appropriate nursing model.
- To undertake activities that promote health, reduce risk, and disease prevention.
