Nurse Practitioner for Community Care

**Job Description:** We are seeking a skilled Community Health Nurse Specialist to join our team at NHS. As a Community Health Nurse Specialist, you will be responsible for delivering high-quality nursing care to patients with complex health needs in a community setting.
**About the Role**In this role, you will work under the supervision of our experienced Community Nurse Team Lead and Deputy Team Lead. Your primary responsibility will be to provide skilled nursing care to patients with long-term conditions, ensuring continuity of care following referral from acute care, rehabilitation units, GPs, and other agencies.
**Key Responsibilities:**
- To deliver high-quality nursing care based on principles of best practice and clinical evidence, including chronic disease management, end-of-life care, continence promotion, and wound management.
- To undertake comprehensive assessments of patients' physical, psychological, and social care needs, including carers' needs, and develop individualised care plans that promote self-care and prevent unnecessary hospital admissions.
- To act as a patient advocate, facilitating patients' choices regarding nursing care and promoting independence and self-care.
**Requirements:**
- Registered Nurse (RN) qualification with NMC registration.
- Evidence of post-registration education and training.
- Recent clinical nursing experience in a multidisciplinary team environment, with a focus on community nursing.
- Knowledge of out-of-hospital care, infection control, general wound care, and recent government legislation changes in community care delivery.
