We are seeking a dedicated Care Coordinator to join our Kent Complex Care team. As a Care Coordinator, you will play a key role in supporting our clients to maintain their health and independence.

Key Responsibilities:

  • Manage client caseloads and collaborate with clients, families, carers, and the wider coordination team.
  • Work 37.5 hours per week, covering shift requirements for our clients.
