Job Title: Complex Needs Support Coordinator

We are seeking an experienced and compassionate Complex Needs Support Coordinator to join our team at Apex Housing Association. This is a fantastic opportunity to make a meaningful difference in the lives of our residents and contribute to their overall well-being.

About the Role:
In this role, you will provide high-quality care and support to our residents, ensuring their physical, emotional, and social needs are met. You will work collaboratively with our team to develop individualized plans, promoting independence, dignity, and empowerment.

Key Responsibilities:
  • Provide personalized care and support to residents with complex needs.
  • Develop and implement individualized care plans, working closely with residents, families, and healthcare professionals.
  • Maintain accurate records and reports, adhering to regulatory requirements.
  • Collaborate with the multi-disciplinary team to ensure seamless delivery of care services.
  • Contribute to a safe and respectful environment, promoting resident autonomy and choice.

Requirements:
  • A minimum of 1 year's experience in a support role, working with adults with learning difficulties and challenging behavior.
  • QCF/NVQ Level 2 Health & Social Care or equivalent qualification.
  • Current registration with NISCC or willingness to register within 6 months.
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse stakeholders.
  • Previous administration experience, with proficiency in IT and report writing.
