We are currently recruiting for a Support Worker to join our team in Derry/Londonderry. In this fulfilling role, you will have the opportunity to make a real difference in the lives of vulnerable adults, promoting their independence and well-being.

About the Role
In this critical position, you will work closely with patients to address their physical, emotional, and social needs. Your exceptional communication skills and ability to build strong relationships will enable you to deliver person-centered care and support.

Responsibilities
- Develop and implement personalized care plans for patients
- Deliver high-quality care and support to patients
- Collaborate with multidisciplinary teams to achieve optimal patient outcomes

Requirements
- NVQ Level 2 or equivalent qualification in Health and Social Care (desirable)
- Minimum 6 months healthcare experience in the UK
- Evidence of full immunity to Hepatitis B, MMR, Tuberculosis & Varicella
- Right to work in Northern Ireland without restriction
