We are seeking an enthusiastic and dedicated Care Coordinator Administrator to join our team at Harpenden Health PCN. This part-time permanent role will involve working between 18-22 hours per week with flexibility around school hours (if applicable).
Key Responsibilities- Establish strong working relationships with GPs and practice teams, working collaboratively with other Care Co-ordinators, Social Prescribing Link Workers, and Health and Wellbeing Coaches.
- Act as a champion for personalised care and shared decision making within the PCN, assisting in monthly MDT meetings and establishing and maintaining links with our Care Homes.
- Liaise with our Clinical Pharmacist teams to ensure patients receive timely and appropriate medication reviews.
- Ensure regular and effective communication with our local support agencies within and outside the NHS.
- Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role.
- GCSE level (or equivalent) including Maths & English.
- Experience of working within multi-professional team environments.
- Excellent communication skills (written and oral), clear polite telephone manner.
- Good knowledge of MS Office and Outlook.
- Strong organisational skills, including planning, prioritising, time management, and record keeping.
- Good interpersonal skills and the ability to work flexibly and enthusiastically within a team or on own initiative.
