Job Summary
Primary Care Careers is seeking a highly motivated and proactive Care Coordinator to join our team on a part-time basis. The successful candidate will be responsible for coordinating key activities, including access to services, advice, and information, ensuring timely, efficient, and patient-centered health and care planning.
Main Duties
- Supporting a cluster of three practices, the Care Coordinator will liaise verbally with patients who have complex health needs and members of the local voluntary sector.
- Coordinating patients' journeys through primary care, ensuring seamless transitions and effective communication.
- Supporting digital initiatives and contributing to the development of innovative solutions.

Job Responsibilities
The Care Coordinator will be responsible for:
- Coordinating patient care plans, ensuring timely and effective implementation.
- Developing and maintaining relationships with patients, carers, and healthcare professionals.
- Providing administrative support to the clinical team, ensuring efficient and effective communication.
Person Specification
The ideal candidate will possess:
- Excellent organizational skills, with the ability to prioritize tasks and manage multiple projects.
- Confident IT skills, with experience of MS Office packages.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
