Care Coordinator Administrator

About the Role
We are seeking a highly motivated and enthusiastic Care Coordinator Administrator to join our team at Harpenden Health PCN. As a Care Coordinator Administrator, you will play a vital role in supporting our clinical teams to deliver high-quality care to our patients.
Key Responsibilities
- Establish strong working relationships with GPs and practice teams, working collaboratively with other Care Co-ordinators, Social Prescribing Link Workers, and Health and Wellbeing Coaches.
- Act as a champion for personalised care and shared decision making within the PCN, assisting in monthly MDT meetings and establishing and maintaining links with our Care Homes.
- Liaise with our Clinical Pharmacist teams to ensure patients receive timely and appropriate medication reviews.
- Ensure regular and effective communication with our local support agencies within and outside the NHS.
- Undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.
Requirements
- Experience of working within multi-professional team environments.
- Excellent communication skills, including written and oral communication, and a clear and polite telephone manner.
- Good knowledge of MS Office and Outlook.
- Strong organisational skills, including planning, prioritising, time management, and record keeping.
- Good interpersonal skills and the ability to work flexibly and enthusiastically within a team or on own initiative.
