Care Coordinator Administrator

We are seeking a highly motivated and organized Care Coordinator Administrator to join our team at Harpenden Health PCN. As a Care Coordinator Administrator, you will play a vital role in supporting our clinical teams to deliver high-quality patient care.
Key Responsibilities- Establish strong working relationships with GPs and practice teams to ensure seamless communication and collaboration.
- Act as a champion for personalized care and shared decision making within the PCN, assisting in monthly MDT meetings and establishing links with care homes.
- Support the work of clinical leads in delivering a holistic and timely support in relation to the NHS Enhanced Service and priorities of the wider NHS England.
- Liaise with clinical pharmacists to ensure patients receive timely and appropriate medication reviews.
- Ensure regular and effective communication with local support agencies within and outside the NHS.
- GCSE level (or equivalent) including Maths & English.
- Experience of working within multi-professional team environments.
- Excellent communication skills (written and oral), clear polite telephone manner.
- Good knowledge of MS Office and Outlook.
- Strong organizational skills, including planning, prioritizing, time management and record keeping.
