Job Summary:

We are seeking a highly skilled Diabetes Specialist Nurse to join our team at Newton Place Surgery. As a key member of our healthcare team, you will be responsible for providing high-quality, evidence-based diabetes care to patients with diabetes from diagnosis to long-term management.

Key Responsibilities:

  1. Autonomous Practice: Work independently to provide specialist diabetes care, utilizing your expertise to develop and implement individualized care plans for patients with complex diabetes needs.
  2. Insulin Management: Initiate and support insulin management and/or other injectables for patients with type 1 and 2 diabetes, ensuring optimal glucose control and minimizing complications.
  3. Patient Education: Promote self-care management to patients, empowering them to take an active role in preventing the worsening of their condition and improving health outcomes.
  4. Administrative Support: Liaise with administrative staff to ensure the smooth operation of recall systems, clinics, and other administrative tasks.
  5. Data Management: Maintain accurate and up-to-date records, using templates and software to ensure compliance with quality and outcomes frameworks, such as the Quality and Outcomes Framework (QOF).
  6. Record Keeping: Maintain accurate and timely records of patient consultations, treatments, and progress, using electronic medical records and other applicable software.
  7. Quality Improvement: Actively collect and maintain data to demonstrate patient improvement in key areas, ensuring that targets are met and quality standards are maintained.
  8. Specialist Advice: Provide specialist advice and guidance to patients, their family members, carers, and healthcare professionals, ensuring that they have access to the best possible care and support.
  9. Multidisciplinary Teamwork: Participate in multidisciplinary team meetings, gathering information and providing updates on patient progress towards goals as per their care plan.
