About the Role

We are seeking a highly skilled and experienced Diabetes Nurse Specialist to join our team at Newton Place Surgery. As a specialist in diabetes care, you will be responsible for providing high-quality, evidence-based care to patients with complex diabetic needs.

Key Responsibilities
  • Work autonomously in the provision of specialist, evidence-based diabetes care for people with diabetes from diagnosis to long-term management.
  • Initiate and support insulin management and/or other injectables to complex type 1 and 2 diabetic patients.
  • Promote self-care management to patients to prevent worsening of their condition and improve health outcomes.
  • Liaise with administration staff regarding recall systems and ensure the smooth running of clinics.
  • Ensure accurate record keeping, recording of all consultations and treatments in patients' medical records using templates where applicable.
  • Maintain accurate data and records in a timely fashion, using EMIS and other software as applicable.
  • Ensure full understanding of QOF for diabetes chronic disease management and liaise with the team to ensure targets are achieved.
  • Act as a specialist resource and provide specialist advice in relation to diabetes, to patients, their family/carers and health professionals.
Person Specification

We are looking for a candidate with:

  • Post-registration experience in a Diabetes Specialist Nurse or practice nurse role with experience running Diabetes clinics independently.
  • Level 1 NMC Adult Registered Nurse qualification.
  • Relevant post-registration qualification.
  • Diabetes Diploma/Certificate.
  • Desirable: Non-Medical Independent Prescriber or willingness to undertake.
