Paediatric Care Coordinator

The Kent Community Health NHS Foundation Trust is seeking a highly skilled and compassionate Children's Community Nursing Specialist to join our team of dedicated healthcare professionals. As a key member of our community children's nursing team, you will play a vital role in providing high-quality, patient-centered care to children and young people with complex health needs.
Main Responsibilities- Provide expert nursing care to children and young people with a range of health conditions, including palliative care, oncology, and respiratory conditions.
- Work collaboratively with multidisciplinary teams to develop and implement individualized care plans that meet the unique needs of each child and family.
- Conduct thorough assessments and evaluations to identify the needs of children and young people, and develop strategies to address these needs.
- Provide education and support to children, young people, and their families to promote healthy lifestyles and self-management of chronic conditions.
- Collaborate with community-based services to ensure seamless transitions of care and optimal outcomes for children and young people.
- Registered Nurse (Child Branch) or RSCN with a strong background in paediatric nursing.
- At least 2-3 years post-registration clinical experience working with children and their families.
- Wide range of professional knowledge and experience of paediatric nursing, including palliative care, oncology, respiratory, cardiac, gastroenterology, special needs, complex care, and experience of working with children/young people with additional needs.
- Sound nursing knowledge and experience of all basic clinical nursing skills, including care of Tracheostomies, enteral feeding devices, and Central venous catheters.
- Good communication skills, including communication with children/young people and their carers, and ability to prioritize workload.
