Children's Community Nurse

We are seeking a highly skilled and compassionate Children's Community Nurse to join our team at Kent Community Health NHS Foundation Trust. As a key member of our community children's nursing team, you will provide high-quality, patient-centered care to children and young people with complex health needs.
Key Responsibilities- Provide comprehensive nursing care to children and young people with a range of health conditions, including palliative care, oncology, and respiratory conditions.
- Work collaboratively with families, carers, and other healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments and evaluations to identify the needs of children and young people, and develop strategies to address these needs.
- Provide education and support to families and carers on how to manage the child's condition and provide ongoing care.
- Participate in multidisciplinary team meetings to discuss patient care and develop care plans.
- Registered Nurse (Child Branch) or RSCN qualification.
- At least 2-3 years post-registration clinical experience working with children and their families.
- Wide range of professional knowledge and experience of paediatric nursing, including palliative care, oncology, and respiratory conditions.
- Sound nursing knowledge and experience of all basic clinical nursing skills, including care of Tracheostomies, enteral feeding devices, and Central venous catheters.
