We are seeking an experienced Social Worker to join our team at Rhodes Wood Hospital, a specialist Inpatient Eating Disorder service for children and young people aged 12-18 years. As a Social Worker, you will play a vital role in supporting our service users to achieve their goals and live fulfilling lives.

Your Key Responsibilities

  • ">
  • Provide social care assessments, risk assessments, and facilitate group activities to support service users in achieving their goals.">
  • Contribute to ICR meetings, working collaboratively with colleagues to develop effective plans for service users.">
  • Liaise with local authorities and other stakeholders to ensure seamless care coordination during admission and discharge processes.">
  • Complete full annual social care reassessments within the CPA format.">
  • Facilitate access to welfare rights, housing, advocacy services, and associated support services.">
