Voyage Care seeks a dedicated and detail-oriented Care Coordination Specialist to assist individuals with autism in achieving their goals and living independently.
This role requires someone who is organized, efficient, and possesses excellent time management skills.
As a Care Coordination Specialist, you will work closely with a team to ensure seamless care delivery and implement effective strategies to meet individual needs.
