NHS Scotland is a leading healthcare provider seeking an experienced Community Hospice Nurse Practitioner to join our team.

Job Description:

The post holder will work as a member of the Community and In-Patient based Palliative Care Team at St Margaret of Scotland Hospice. The successful candidate will have the skills and experience of working and promoting Palliative Care within a Hospice and/or community or Hospital setting.

Key Responsibilities:
  1. To work closely with members of the multi-professional team within St Margaret of Scotland Hospice and the wider community projects/services to provide specialist palliative care.
  2. To participate in daily Ward Rounds, weekly Grand Ward Rounds, and daily Patient handovers.
  3. To establish oneself as a resource, facilitator, and role model for Specialist Palliative Care.
  4. To adopt a multi-professional and collaborative team approach to care assessment and delivery.
  5. To support families/carers in bereavement.
Salary:

Estimated salary range £45,000 - £60,000 per annum, depending on experience.
