Patient Care Coordinator

We are looking for a dedicated and compassionate Patient Care Coordinator to join our team. As a key member of our care team, you will work closely with registered nurses to deliver high-quality patient care in a dynamic hospital environment.
About the Role- Support patients with personal care, hygiene, and sanitary needs while maintaining confidentiality.
- Contribute to the development of care plans and implement them under the guidance of registered nurses.
- Communicate effectively with patients, families, and healthcare professionals to ensure smooth care delivery.
This role requires someone who is empathetic, organized, and able to work well in a team environment. Previous experience in care settings or holding an NVQ/QCF Level 2 Diploma in Clinical Healthcare Support is advantageous.
Main ResponsibilitiesKey Responsibilities- Provide high-quality patient care, prioritizing patients' needs and preferences.
- Support registered nurses in taking and recording vital signs as per the patient's plan of care.
- Participate in education and training programs to enhance your skills and knowledge.
