Join our NHS team as an Adult Safeguarding Care Coordinator and play a vital role in ensuring the well-being of adults at risk. With a Foundation Degree in Integrated Care or equivalent, you will work collaboratively with healthcare professionals to implement bespoke support plans.

The estimated salary for this position is £26,000 - £32,000 per annum, depending on experience. In addition to a competitive salary, we offer excellent terms and conditions, including optional membership to a contributory pension scheme and a full induction program.

Key Responsibilities:

  • Support patients in a 24-hour period, ensuring their physical, social, and wellbeing needs are met
  • Work with a multidisciplinary team (MDT) and healthcare professionals to implement bespoke plans of support
  • Admit and discharge patients, provide staff support and supervision, and perform clinical and social care duties
  • Develop and maintain accurate and legible records

Requirements:

  • Foundation Degree in Integrated Care or equivalent
  • Level 3 in health or social care with willingness to work towards level 4 on appointment
  • Maths and English Level 2 qualification or equivalent level of competence
  • Certificate in administration of medication or equivalent
  • Experience in Microsoft Office applications: Word, Excel, Outlook, and Access
  • Minimum 2 years proven experience of working in a health and/or social care environment working effectively as part of a team
  • Ability to maintain confidentiality, demonstrate tact and diplomacy, and assist patients with social, personal, and physical care needs
