We are recruiting for a highly skilled Home Care Coordinator to join our Emergency Duty Team in Grimsby, Lincolnshire. The ideal candidate must have care experience and the ability to work independently.

The role involves answering emergency telephone calls from service users, their family members, Care Managers, and our Care Workers in the local area, providing support with queries and ensuring all necessary information is documented and reported.

You will also be required to cover care in the event of staff sickness, collaborate with other workers, and may need to provide emergency care in the community yourself.

Key responsibilities include:

  • Ensuring accurate documentation and reporting of calls, incidents, and sickness.
  • Logging all incoming calls and documenting actions taken.
  • Assigning care to care workers using our electronic care planning system.
  • Managing medication on service users' electronic MAR charts and updating planned care in real-time.
  • Dealing with out-of-hours GP, ambulances, local authorities, and safeguarding when required.

A competitive salary of £11.66 per hour awaits the successful candidate.
