Following the Trust's values, this role promotes health and wellbeing of patients in secure environments. Collaborating with the Team Leader, you'll create a safe and caring environment based on individual needs. You'll assess health and social care needs, develop care plans with patients and staff teams, and undertake risk assessments. This position requires you to work with the multi-disciplinary team and relevant agencies to provide effective case management to a defined caseload. You'll ensure effective pre-release care planning and respond to urgent referrals. With evidence-based interventions and proactive risk management approaches, you'll help service users reduce and manage symptoms and high-risk behaviour. You'll also ensure safeguarding procedures are followed and provide advice and information to patients about their care. This role coordinates CPA Review meetings, produces written care plans, and circulates care plans to relevant people and agencies in accordance with Trust policies and guidelines. You'll safely plan patient care back into the community from Prison or transferring to alternative prison establishments, ensuring continuity of care is provided.
