Job Summary:
We are seeking a skilled Homeless Outreach Worker to join our team at Brook Street Social Care NMR. As a key member of our outreach team, you will be responsible for supporting individuals who are homeless and living on the streets, or at risk of becoming homeless. Your primary goal will be to encourage them to access supported accommodation and provide person-centred support to help them secure suitable housing and meet their immediate needs.

Key Responsibilities:
• Promote independence and encourage positivity and healthy lifestyles among service users.
• Work with individuals who have complex needs, including drug and alcohol abuse and mental health issues.
• Collaborate with other agencies to provide comprehensive support to individuals in need.
• Make quick decisions to help individuals or refer them to external agencies as necessary.

Requirements:
• A background in supporting adults who are homeless or at risk of becoming homeless.
• A UK driving licence is essential for this role.
• A current DBS on the update service and eligibility to work in the UK are also required.
