Job Description
We are seeking an experienced Customer Service Navigator to join our team. As a key member of our primary care team, you will be responsible for providing care navigation call handling duties in line with PC24 processes. This will involve communicating with patients, supporting the wider primary care team, and actioning follow-up calls. You will also provide operational support to the onsite practice team and clinical team, ensuring patient safety by following correct processes.

Key Responsibilities

Requirements
To be successful in this role, you will need:
* GCSE Level C in literacy and numeracy or equivalent work-based experience
* Excellent communication skills with a wide variety of people
* Excellent keyboard skills and experience of Microsoft Office packages
* Patient-focused skills with the ability to show patience, empathy, and compassion via telephone
